Voor het invullen van attesten en formulieren is informatie en tijd nodig. Daarom dient u hiervoor op raadpleging te komen. Deze kunnen niet langer telefonisch of per mail aangevraagd worden. Uw arts mag enkel attesten schrijven die stroken met de waarheid. Valse attesten zullen steeds geweigerd worden.
Voorschriften worden nog uitsluitend afgeleverd tijdens een raadpleging. Vraag daarom tijdens je raadpleging voldoende medicatievoorschriften aan uw arts.
